Hourly rates
- On weekdays: $110/hr regular rate except long distance (see below)
- On weekends and holidays: $180/hr
- Long distance: $130/hrfor hospitals that are 45 min drive or more from Wilmette, this includes all hospitals within Chicago city limits
- Lower rates may apply when payed via W-2 tax form upon mutual agreement
Cancellation policy
- No charge if cancelled up to 30 days prior to requested date
- 50% charge of arranged shift if cancelled within 30 day
Billing
- Payment is expected within 2 weeks of the invoice, unless other arrangements are made prior to my shift.
